COMPANY POLICY
VACCINATION REQUIREMENTS
All puppies are required to provide veterinary proof of up-to-date vaccinations including:

First series of DHPP or DAPP vaccinations

Bordatella (aka Kennel Cough) vaccination

(Optional) Canine Influenza if recommended by your veterinarian*
All dogs over 6 months are also required to provide:

Rabies vaccination

Veterinary proof of up-to-date distemper and bordatella vaccinations
* Happy Tailz Dog Training is not a veterinary service and will defer to your veterinarian's advice regarding additional vaccinations.
SAFETY & LIABILITY
- A Happy Tailz Dog Training liability waiver is required to be signed for all on-site training classes prior to the first session.
- A Happy Tailz Dog Training liability waiver is required to be signed for all on and off-site elements of Day Camp classes prior to the first session.
- A Happy Tailz Dog Training liability waiver and emergency treatment release forms are required to be signed prior to any Day Camp class or off-site boarding reservation.
CANCELLATION & REFUNDS
If you're not completely satisfied with your training experience, you can request a full refund at any time prior to your 3rd scheduled training session. If you have questions or concerns about any issue during training, please do not hesitate to speak with our trainer. We will do our very best to solve any problems and make your training experience as positive and productive as possible. You and your dog's happiness is always our primary goal.
